About Caroline Graham, MSN, FNP-C, PMHNP-BC
Serving The State of Oregon
Caroline is a dual-certified Family Nurse Practitioner and Psychiatric-Mental Health Nurse Practitioner. She began her nursing career in 2009. Motivated by a desire to take a more active role in patient care, she became a nurse practitioner in 2018. Her early work focused on holistic, integrative medicine and later grew to include substance use treatment and co-occurring conditions, where she found her passion for mental health.
Her training and experience in caring for individuals with a broad range of mental health concerns including trauma, depression, anxiety, and ADHD has led her to develop a clinical approach that integrates trauma-informed care, psychiatric medication management, and mind-body awareness to address the unique needs of each person.
Caroline is dedicated to creating a safe, inclusive environment where patients feel heard, supported, and empowered. She works collaboratively with individuals to build resilience and achieve meaningful, sustainable wellness.
"I want my patients to feel that they have someone in their corner, someone who is here to help them achieve their goals. My goal is to create a safe, non-judgmental space where they feel truly seen and heard. I honor who they are today, while making room for who they want to become. This is your place to begin the journey toward mental wellness and a fulfilling life, and we'll be with you every step of the way."
